A minimum 6-week notice period before departure is required for us to be able to accommodate your travel request.

Any request submitted less than 6 weeks prior to travelling may be declined.

To help the travel nurses to assess your needs, it is important that you fill out this form before your appointment.

Please enquire with your local Travel Clinic to find out which vaccinations you need for your trip before you complete this form – most pharmacies offer this service.

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• the list of vaccinations the Travel Clinic advised you to get depending on your destination.
